Supporting Documents

Marriage Certificate(s)
Certified copy — one per spouse, if you are married.
Birth Certificates
Certified copies for son/daughter dependants.
Affidavit
Notarized certified copy for non-child dependants (niece, nephew, etc.).
Attestation Letter
Certified letter from your employer confirming employment details.
Tip: All supporting documents must be certified copies. Not all documents are required for every member — the system will only ask for what applies to your situation.
